Start with choices, not private facts or records.
You should be able to ask for a review without placing medical details, a case history, an account number, or another sensitive record into a website form. The initial request uses broad, closed choices and contact information only.
What the initial request asks for
The form asks for your name, how to reach you, a broad incident category, roughly when it happened, and the question you want Jason to consider. It does not accept a written case summary or uploads. Jason can ask for facts directly if a conversation would help.
When you see a confirmation number
A confirmation number appears only after the website receives the required choices and your permission to contact you. It confirms receipt of the request—not acceptance of a matter or an attorney-client relationship.
Your choice about information on site use
Optional information about how this site is used stays off unless you allow it on this device. If allowed, it may include a random identifier, time, consent choice, page, form location, and permitted campaign or referring-site information. It excludes names, phone numbers, email addresses, and form summaries.
Permission to contact you is separate
Permission to contact you applies only to the submitted review request using the information you provide. It does not authorize marketing messages. Optional information about site use is a separate device choice.
Where a request goes and how it is kept
The website uses Netlify to host and process a review request. The site operator and its service providers may access it only for intake, security, maintenance, or legal obligations. Ordinary inquiry records are scheduled for deletion after 90 days unless a valid legal hold applies.
